PUMP INQUIRY FORM INSTRUCTIONS - PLEASE READ CAREFULLY:
This inquiry form is designed to cover a wide range of applications and pump types, therefore some questions may not apply to your application. Please use one form per application, provide as many details as possible and leave the other areas blank or if you prefer enter N/A for non-applicable. You need to provide all the requested contact information, otherwise your inquiry may be delayed or not processed. Once you select “CONTINUE” at the bottom of each screen you cannot go back to review or correct the data, therefore you need to ensure the information provided on each screen is correct before continuing to the next screen. After entering the image verification code and submitting your inquiry, you will automatically be returned to the first page where a message will be posted in red just above the form confirming "Your information has been sent successfully". You can continue with your next application. An email will be sent within 24 hours confirming receipt of each inquiry. If after submitting you realize an error was made, please send a new inquiry form, with all the contact information, reference the original application identification, fill in only the fields that need to be corrected, then confirm/clarify these changes in the "Other Specifications" box at the end of the form. |
